You should also consider that Lemmy is beta software and in heavy development stage. Things like this could easily happen in an instance without moderators noticing of the issues.

Despite being “decentralized,” things are still unilateral at an unprecedented amount and in a centralized way in many communities. If there’s one community moderator, it’s their community. If you delete your comment, they can un-delete it.
Administration works similarly; a community can be viewed across many servers but it’s controlled on only one (welcome to lemm.ee). That gives the admin veto power over any moderators from any server, despite where they’re from.
